Pooled trial data show berberine reducing HbA1c by about 0.63 percentage points and fasting glucose by about 0.8 mmol/L in type 2 diabetes — a real effect, larger than any other supplement reviewed here. The literature is dominated by trials from a single region with known quality concerns, and berberine has genuine drug interactions. It is not a supplement to start without telling your prescriber.

What the trials found

A 2022 meta-analysis pooled 37 randomised trials covering 3,048 people with type 2 diabetes. The effect sizes are not trivial: reductions on the order of a modest pharmaceutical.

The same analysis reported no increase in total adverse events or in hypoglycaemia risk when berberine was used alone or alongside oral glucose-lowering drugs — though that is a different question from whether it is safe alongside every other medication someone might be taking.

Why we tier this as emerging, not strong

The pooled figures look impressive, and the evidence base underneath them is not. That analysis searched four Chinese-language databases alongside the international ones, and the berberine trial literature is heavily concentrated in trials published there — a body of work with documented concerns about methodological quality and publication bias.

This is not a claim that the trials are wrong. It is a statement about how much a pooled average from that literature can be relied on, and the honest answer is: less than the confidence interval suggests. Larger, independently run trials would settle it.

The interaction problem

Berberine inhibits intestinal P-glycoprotein, the transporter that limits how much of certain drugs get absorbed. Block it and those drugs reach higher levels than prescribed. In animal work, digoxin and ciclosporin exposure both rose substantially — two drugs where the gap between therapeutic and toxic is narrow.

Our audience is the reason this matters more here than on a general wellness site. People reading about glucose supplements are disproportionately people already taking glucose-lowering drugs, and berberine's effect is additive with all of them.

Is berberine as good as metformin?
Some trials compare the two directly and report similar glucose reductions, but they are small and mostly from one literature. Metformin has decades of outcome data behind it and berberine does not. They are not interchangeable, and swapping one for the other is a decision for a prescriber.
Can I take berberine with metformin?
Their effects on glucose add up, which can push you lower than either dose intended. That is a conversation to have with whoever prescribed the metformin before you start, not after.
How long does the effect last?
Most trials in the pooled analysis ran for a few months. There is little long-term data, and no trial has shown that berberine reduces the complications that matter — only the numbers that predict them.
