Réponse courte
Does berberine lower blood sugar?
Pooled trial data show berberine reducing HbA1c by about 0.63 percentage points and fasting glucose by about 0.8 mmol/L in type 2 diabetes — a real effect, larger than any other supplement reviewed here. The literature is dominated by trials from a single region with known quality concerns, and berberine has genuine drug interactions. It is not a supplement to start without telling your prescriber.
Avant d’en prendre
Parlez-en à votre prescripteur avant d’ajouter ceci à un médicament, en particulier hypoglycémiant. N’arrêtez jamais un traitement prescrit au profit d’un complément.
Interactions
Additive with glucose-lowering medication. Taken alongside metformin, insulin or a sulfonylurea, berberine can push glucose lower than either was meant to — a hypoglycaemia risk that rises with the drugs most likely to be in our readers' cabinets.1
À éviter
Inhibits intestinal P-glycoprotein, raising blood levels of drugs that depend on it. In animals, berberine increased digoxin exposure by up to 70% and ciclosporin exposure by up to 96%. Both have narrow margins between a working dose and a toxic one.2
À éviter
Modelling that accounts for both CYP3A and P-glycoprotein inhibition predicts high-dose berberine (300 mg three times daily) will raise exposure to some CYP3A substrates. These are simulations, not measured human interactions, and the authors call for controlled studies.3
Prudence
Qui ne doit pas en prendre
Not for use in pregnancy or while breastfeeding. Berberine crosses the placenta and is excreted in breast milk, and it displaces bilirubin from albumin — the mechanism behind kernicterus in newborns.
À éviter
Not for infants or young children, for the same reason.
À éviter
If you take any prescription medicine at all, talk to your prescriber before starting berberine rather than after. This is the supplement on this site with the most real interaction potential.
Prudence
Notre verdict
The strongest evidence of anything on this page, from the weakest evidence base — and with real interaction risk attached. Worth discussing with a prescriber; not worth starting quietly.

What the trials found
A 2022 meta-analysis pooled 37 randomised trials covering 3,048 people with type 2 diabetes. The effect sizes are not trivial: reductions on the order of a modest pharmaceutical.
The same analysis reported no increase in total adverse events or in hypoglycaemia risk when berberine was used alone or alongside oral glucose-lowering drugs — though that is a different question from whether it is safe alongside every other medication someone might be taking.
Ce que disent les preuves
Across 37 randomised trials in 3,048 people with type 2 diabetes, berberine reduced HbA1c by 0.63 percentage points, fasting glucose by 0.82 mmol/L and two-hour post-load glucose by 1.16 mmol/L.1
Preuves émergentes
The size of the effect depended on how high the participant's baseline glucose and HbA1c were — the benefit concentrates in people whose numbers were worst to begin with.1

Why we tier this as emerging, not strong
The pooled figures look impressive, and the evidence base underneath them is not. That analysis searched four Chinese-language databases alongside the international ones, and the berberine trial literature is heavily concentrated in trials published there — a body of work with documented concerns about methodological quality and publication bias.
This is not a claim that the trials are wrong. It is a statement about how much a pooled average from that literature can be relied on, and the honest answer is: less than the confidence interval suggests. Larger, independently run trials would settle it.
Ce que disent les preuves
The pooled berberine evidence draws substantially on trials indexed in Chinese-language databases, a literature with recognised quality and publication-bias concerns.1

The interaction problem
Berberine inhibits intestinal P-glycoprotein, the transporter that limits how much of certain drugs get absorbed. Block it and those drugs reach higher levels than prescribed. In animal work, digoxin and ciclosporin exposure both rose substantially — two drugs where the gap between therapeutic and toxic is narrow.
Our audience is the reason this matters more here than on a general wellness site. People reading about glucose supplements are disproportionately people already taking glucose-lowering drugs, and berberine's effect is additive with all of them.
Ce que disent les preuves
Berberine increased digoxin exposure by up to 70% and ciclosporin exposure by up to 96% in animal studies, through inhibition of intestinal P-glycoprotein.2
Preuves émergentes
The same animal work found no significant change in CYP3A activity, while pharmacokinetic modelling predicts clinically relevant CYP3A-mediated interactions at high berberine doses — the two lines of evidence do not fully agree.2,3

Questions fréquentes
- Is berberine as good as metformin?
- Some trials compare the two directly and report similar glucose reductions, but they are small and mostly from one literature. Metformin has decades of outcome data behind it and berberine does not. They are not interchangeable, and swapping one for the other is a decision for a prescriber.
- Can I take berberine with metformin?
- Their effects on glucose add up, which can push you lower than either dose intended. That is a conversation to have with whoever prescribed the metformin before you start, not after.
- How long does the effect last?
- Most trials in the pooled analysis ran for a few months. There is little long-term data, and no trial has shown that berberine reduces the complications that matter — only the numbers that predict them.
