What actually happens after a meal

Trials giving vinegar with a carbohydrate meal consistently record a smaller glucose excursion and a smaller insulin response than the same meal without it. The pooled effect is moderate for glucose and larger for insulin.

The mechanism is not mysterious: acetic acid slows gastric emptying, so the same carbohydrate arrives in the bloodstream over a longer period. That is a genuine physiological effect and it is also why it is contraindicated in gastroparesis.

The gap between the curve and the outcome

This is the clearest example on the site of a measurable change that has not been shown to matter. Every finding above is about the hours after a meal. None of it demonstrates that vinegar with dinner every night for a year lowers HbA1c, reduces weight, or prevents anything.

The reason it deserves a place here anyway is that a sensor makes the short-term effect visible, and seeing a curve flatten is persuasive in a way that a null long-term literature is not. Being persuaded by the thing you can see, rather than the thing that was measured, is the error this whole education section exists to head off.

Dilute it — a tablespoon in a large glass of water, with food, not neat and not on an empty stomach. Undiluted vinegar strips tooth enamel and has caused oesophageal burns, and the tablet forms have their own history of lodging in the throat.

A vinegary salad dressing with the meal does the same job and puts your teeth at less risk than a shot glass does.

The active component in the trials is acetic acid, which every vinegar contains. Nothing in the evidence singles out the apple cider variety; that is branding.
When should I take it?
With the meal, since the effect is on how that meal is absorbed. Taken hours apart from food there is no mechanism for it to do anything.
Will it show up on my CGM?
Probably, if your sensor samples often enough to resolve the shape of a meal response. A device reading every 15 minutes may not show a difference that a one-minute device would.
